Donor Athlete Sponsorship Form

Are you a living donor athlete or transplant center personnel looking for financial sponsorship for your next competitive athletic event? Donor Games may be interested in sponsoring you.

The National Kidney Registry and its Donor Games initiative will consider applications for event sponsorship from living donor athletes and transplant center personnel for a wide variety of competitive events including, but not limited to, triathlons, endurance races, CrossFit competitions and more.

Please complete the form below to be considered for sponsorship. All forms must be completed at least 60 days prior to the event start date. The Donor Games requires at least 30 days from receipt of application to make a decision on each application. If approved for financial sponsorship, we request at least 30 days prior to the event to promote the athlete and his/her event.

NKR Sponsored Athlete Terms and Conditions

The Undersigned Sponsored Athlete (“You”) hereby agrees to all of the following National Kidney Registry, Inc. (“NKR”) Sponsored Athlete Terms and Conditions, in conjunction with a competitive athletic event (“Event”).

  • 1. SPONSORSHIP APPROVAL AND PAYMENT: Submission of an application does not guarantee sponsorship. If you are approved for sponsorship, NKR will determine the amount, timing, and method of sponsorship payment or reimbursement in its discretion and will communicate those details to you.
  • 2. FUNDRAISING RESTRICTIONS: You may not utilize any other peer-to-peer fund-raising mechanisms, including but not limited to GoFundMe.
  • 3. LOGOS AND BRANDING: You shall not utilize, wear, or otherwise display in any manner, any other logos or brands of any competing organ donation, transplant, paired exchange, or donor advocacy organizations, without NKR’s prior consent. You shall not use, publish, or reproduce the NKR logo, or any other intellectual property owned or controlled by NKR, or any similar names, trademarks, or trade names of NKR, in any marketing, advertising or promotion materials, without NKR’s prior consent. This Agreement constitutes NKR’s approval to use the NKR name and logo solely in connection with the Event and this sponsorship.
  • 4. PUBLICITY, SOCIAL MEDIA, AND LIKENESS: You shall provide NKR with 3 paragraphs of biographical information, no less than 2 general photos of you, no less than 5 professionally captured photos of you, for each day of the Event. NKR reserves the right to request specific photos of you during the Event, which you shall provide within 3 (three) days of completion of the Event. You shall also provide us any and all raw video footage taken of you, within 3 (three) days of our request. You grant us a perpetual, worldwide, royalty-free, transferable, non-exclusive license to use, reproduce, publish, distribute, display, modify, and create derivative works from your name, image, likeness, biographical information, photographs, and videos for promotional, educational, and other business purposes. You may use photos and videos that are posted to your social media outlets during the event, 30 days before, and 30 days after, in which you properly “tag” the NKR. When posting about NKR, the Event, or this sponsorship on social media or other public platforms, you agree to clearly disclose your sponsorship relationship with NKR in accordance with any applicable laws and guidelines. NKR may request removal of content that NKR reasonably determines is inconsistent with its mission, brand standards, sponsorship requirements, or applicable law.
  • 5. MEDIA AND PRESS: In the event you are contacted by the press or media, you may disclose the nature of this sponsorship. You are required to provide us a copy of any published media, written, video, or otherwise.
  • 6. SPONSORSHIP WITHDRAWAL AND REPAYMENT: NKR reserves the right to withdraw this sponsorship, in their sole discretion, in the event of any breach of this Agreement, including but not limited to utilizing the logos of a competitor. If you do not participate in the Event, NKR may withdraw this sponsorship. If sponsorship funds have already been paid and you materially breach this Agreement or fail to participate in the Event without NKR’s approval, NKR may require repayment of all or a portion of the sponsorship funds. NKR may also withdraw sponsorship if you engage in conduct that, in NKR’s reasonable judgment, could damage NKR’s reputation or is inconsistent with NKR’s mission and values.
  • 7. ASSUMPTION OF RISK AND RELEASE: Participation in athletic events involves inherent risks, including serious injury, disability, and death. You voluntarily assume all risks associated with participation in the Event. You acknowledge that NKR does not organize, operate, or control the Event and is not responsible for the Event’s safety, conditions, or operations. To the fullest extent permitted by law, you release, waive, and hold harmless NKR and its officers, directors, employees, and agents from any claims arising from your participation in the Event, except to the extent caused by NKR’s gross negligence or willful misconduct.
  • 8. NO EMPLOYMENT RELATIONSHIP: Nothing in this Agreement creates any employment, agency, partnership, or joint venture relationship between you and NKR. If you are a student-athlete, you are responsible for ensuring that participation in this sponsorship complies with your school’s policies and any applicable athletic rules.
